In the first two parts of this series, we’ve explored how the aviation and defence sectors have developed robust frameworks for AI implementation in safety-critical environments, and how UK emergency services are developing their own doctrinal approaches through frameworks like the NFCC’s ethical principles and NHS clinical safety standards. The common thread? AI must augment human decision-making, not replace it.

But what makes AI systems trustworthy enough to support operational crews in high-stakes environments? In this article, we examine the critical elements that underpin effective AI-enhanced Organisational Learning: transparency, fail-safe design, and data quality.

Building Trust: Transparency and Explainability

A consistent theme across all sectors and jurisdictions is that AI systems gain adoption not through sophistication alone, but through transparency and explainability. The UK CAA’s consumer protection framework emphasises that consumers “need clear information about how AI affects their aviation experience,” requiring systems that “communicate the decision-making processes of AI-based systems” in accessible ways.

The US Air Force doctrine similarly emphasises that “transparency and explainability in AI systems are crucial for understanding system behaviour and building confidence.” For Organisational Learning systems in emergency services, this means AI recommendations must include clear reasoning, while also showing which previous incidents, patterns, or data points informed specific suggestions.

When an AI-powered Lessons Management system produces a particular protocol or warning – whether highlighting risks associated with timber-frame construction fires, flagging concerns based on similar custody incidents, or suggesting clinical pathways based on comparable patient presentations – operational personnel need to understand the connection. i.e. Which incidents does this relate to? What patterns triggered this recommendation? How confident is the system in this assessment?

This transparency serves multiple purposes:

  • It builds operational trust in the system across the Police, Ambulance, and Fire Services
  • It enables personnel to validate AI recommendations against their own professional experience and current situational awareness

It supports service governance and accountability, allowing senior leadership to audit how lessons are being identified, shared, and applied across teams, shifts, and divisions

For clinical applications in ambulance services, transparency is particularly critical. Paramedics and clinicians must understand the evidence base behind any AI-suggested clinical pathway or treatment recommendation, maintaining their professional duty of care while benefiting from Organisational Learning.

Fail-Safe Design: When Systems “Don’t Know”

Aviation’s principle of fail-safe design holds particular relevance for Lessons Management across all emergency services. Systems should “default to the safest option when uncertain, alerting humans for intervention” rather than producing unreliable results. The UK MOD’s work on AI also  highlights the need for systems that function safely even when “connectivity to deployed platforms is limited,” requiring clear understanding of “the trade-offs being made” between accuracy and timeliness.

For Organisational Learning systems in emergency services, this means acknowledging uncertainty. If an AI system cannot find relevant previous incidents or lessons for a current situation – perhaps a novel scenario involving new threats, emerging medical conditions, or unprecedented tactical challenges – it should clearly indicate this gap rather than offering tenuous connections.

This “graceful degradation” actually enhances the trust that personnel develop, and encourages confidence that when the system does make recommendations, they’re based on solid evidence from the service’s operational history.

It also highlights knowledge gaps that merit attention. If emerging operational scenarios consistently lack historical precedent – such as incidents involving:

  • Drone interference with emergency operations
  • Novel psychoactive substances affecting patient presentation
  • Cyber-attacks on emergency service infrastructure
  • New construction methodologies affecting fire behaviour
  • Evolving terrorist tactics
  • Emerging infectious diseases

…this signals evolving challenges requiring new operational guidance, training, clinical protocols, or tactical approaches.

Data Quality and Responsible Development

Many sectors emphasise that AI effectiveness depends, fundamentally, on the quality of data. The experience that the US Air Force gained from Project Maven highlighted that “data conditioning is a major challenge, especially when aggregating data across separately designed systems.” The UK MOD Playbook similarly notes challenges with “manually recorded data and service records, with consequent data quality issues around accuracy and consistency.”

For emergency services, this underscores the importance of “consistent incident documentation and lessons capture”. AI systems can only offer relevant Organisational Learning if that learning is properly recorded, categorised, and maintained across:

  • Incident logs and operational debriefs
  • Clinical audit and patient care records
  • Investigation reports and critical incident reviews
  • Near-miss reporting and safety observations
  • Multi-agency debrief outputs

This doesn’t mean perfection however – effective AI can work with imperfect data – but it requires commitment to the systematic and disciplined capture of operational experience.

Interoperability is particularly crucial in emergency services. The NFCC’s emphasis on interoperability by design addresses the need to avoid fragmented systems that cannot share knowledge effectively. For Blue Light services, this principle extends beyond individual organisations:

  • Within services: Enabling a police force in one region to benefit from lessons learned by colleagues elsewhere
  • Across services: Allowing multi-agency learning to flow between police, ambulance, and fire services
  • Across levels: Connecting local, regional, and national learning systems

National consistency in key areas, combined with local innovation, enables Organisational Learning to span individual services while respecting operational autonomy and clinical governance.

Importantly, this requires a “non-punitive learning culture”. NASA’s Aviation Safety Reporting System demonstrates the value of encouraging honest incident reporting without fear of punishment. This cultural change is essential across all emergency services, as AI-enhanced Lessons Management depends on comprehensive data capture, which requires organisational environments that value learning over blame. Services must ensure that debriefs, near-miss reports, critical incident reviews, and clinical audits are captured honestly and completely, feeding the organisational memory that protects future personnel and the public we serve.
